Dear Parents,

 

Welcome to our summer fun camp at Gymnastics Plus. We are happy to have your child/children with us this year. We have an exciting program planned for your children. In order for our program to run efficiently, we have some procedures we would like you to follow. Some changes have been made to this year’s program so be sure to read this booklet carefully for any questions you may have.

 

Hours of operation:

Our program time runs from 7:00am to 6:00pm. Should your child not be attending our camp for the day, please call the office and let us know. The phone number is 769-1155. Your child must be signed in and out each day by an adult, the sign in/sign out sheets are on the counter in the camp lobby. Please sign your name, not mom or dad. You may bring your child anytime after 7:00am. All children must be picked up by 6:00pm to avoid the $1.00 per minute/per child late fee. No child will be allowed to wait in the lobby for pick up. This fee will have to be paid before your child returns to camp.

 

Lunch and snacks:

Your child must bring lunch or money for lunch each day! We do provide morning and afternoon snack for your child, however lunch is your responsibility. You can send your child a packed lunch or buy lunch from the concession stand:

 

Hot Dog, Chips and Drink     PBJ, Chips and Drink    Mac & Cheese and Drink   all are $4.00

 

We do have certain days when other lunches are available:

 

Monday        Chick-Fil-A  6 nuggets, chips, cookie and drink    $6.00

Wednesday   Dairy Queen                                                           $4.00

Friday           Pizza  2 slices, chips and drink                               $6.00

 

The desk person MUST know which lunch your child is buying when you bring them in. Chick-Fil-A and Dairy Queen must be called in by 9:30 and Pizza by 10:00. We cannot order lunches after these times. Because of the number of campers enrolled we will not be able to refrigerate your child’s lunch, therefore it is your responsibility to make sure their lunch is kept cool. You may leave money at the counter to be put in your child’s envelope for lunch and/or snack. This money must be CASH. This may be done on a daily or weekly basis. Please specify what the money is to be used for. Snack prices are between .25 and $1.00, Icee’s are $1.00,drinks are $1.00 and Capri Sun Juices are 75cents. Thursday is ice cream day. Ice cream prices range from .75-$2.50.  Please send money if you want your child to buy ice cream. (On field trip days, you must send your child a packed lunch; we cannot make lunches for your child to bring with them).

 

 

 

                                                                                                                                   

                                                                                                                                   

 

 

 

We prefer that your child not have access to the business telephones (emergencies only) so please make plans for picking your child up, fun nights or sleepovers at a friend’s house in advance. If you call for your child, we can take a message and have them call you back. We cannot call them to the phone.

Nintendo DS, trading cards and cell phones, etc…. are NOT our responsibility. Should your child choose to bring them and they get lost…WE are NOT responsible.

 

Payment Information:

Payments are due on Monday of each week. A late fee of $20.00 will be added if not paid by Wednesday of that week. There are no exceptions to this policy. Summer camp fees are  $85.00 for the first child and $80.00 for each additional child. These fees are due each week regardless if your child attends for that week or not. You do get 1 vacation week during the summer. The registration fee is $50.00 per child and is non-refundable.  If payment for the week is not made by Friday, we will not accept your child on Monday or until full payment with late fees are paid. You will be charged a $50 re-instatement fee.

 

Field Trips:

Field trips are available for an extra fee. Each age group will be invited to go on 1-2 trips each week. On Thursday of each week, we will have our weekly information sheet available to you. This lets you and your child know what we have planned for the next week. It also contains field trip permission forms and payment information. Please read these carefully as they will have a deadline for turning them in. We will not accept payment or forms after this date. Once paid for, field trips are not Refundable or Transferable. All forms and payments must be turned in at the same time.  If your child misbehaves on a field trip he/she will be suspended from the following weeks trips. If your child misbehaves on a second trip they will be suspended from all remaining fields trips.

 

Holidays:

We will be closed on Friday, July 4, 2010 for Independence Day. Should the Gym need to close for adverse weather, a message will be left on the answering machine. It will be the parent’s responsibility to call for information. Fees will remain the same.

 

Behavior Policy.

Our rules are very simple. Please go over these with your child.

  1. Respect staff and other children.
  2. Listen to your teacher and follow the rules. Stay with your color group.
  3. Keep rude and negative remarks, your hands and feet to yourself. There is absolutely no fighting! Tell your teacher if someone is bothering you. Let us handle it; this keeps you out of trouble.
  4. When in the gym, follow the rules. Your teacher will go over these before you start playing. Stay with your group at all times.
  5. Take care of all supplies, games and equipment.
  6. Leave things alone that do not belong to you. Stay out of other children’s cubbies.
  7. When on a field trip, stay with your group and listen carefully to the staff. You must wear your Gymnastics Plus T-shirt.
  8. There is no running while you are indoors. Please save your running for outside activities.
  9. Put all of your belongings in your cubby. This includes your shoes when you go to the gym, lunch box etc…

 

 

  1. Discipline policy is as follows:

 

1st – 2nd offense your child will receive a parent alert and write sentences.

3rd  offense your child will receive 3 days suspension in which you are still required to

       pay their fee.

4th offense your child will receive 1 week suspension in which you are still required to

      pay their fee.

5th offense your child will be terminated from our program.

 

As an extra to you and your child enrolled full time we have included a gymnastics or tumbling class for your child to participate in once a week. This class is for levels 1 and 2. As your child moves up in our program, you will be responsible for paying the full amount of their tuition for those classes. Those classes are limited in size due to the training they receive. Examples would be Level 3, Tumble Katz, Developmental boys, Training team, and team. At that point your child has entered into the competitive side of our program and are receiving specialized training. If you choose for your child not to move up, they will stay in their present class. If you would like for your child to take an extra level 1 or 2 class or tumbling class you will receive a discount price. *****

 

The annual registration fee will be $20.00.                                 Regular fee is $50.00.

 

1 extra day Gymnastics Class is $40.00 per month.                   Regular fee is $45.00

1 extra day Tumbling Class is    $40.00 per month.                    Regular fee is $45.00

 

If for any reason you should have concerns or questions about our program please feel free to call Mrs. Paula at the Lynn Haven gym. If you feel your questions have not been answered then you can leave me a voice mail and I will get back with you as soon as possible.

 

We require a 1-week notice when leaving our program.

Discipline Policy

 

Children will not be subjective to harsh or cruel treatment, abusive language, or punishment, or punishment associate with work, food or napping.

 

No spanking of any child will ever be allowed or permitted in our center.

 

No touching, tickling, or manhandling is ever permitted in our center.

 

The time out method will be used for the disciplining of children. If this method fails parents will be notified. If a child’s behavior cannot be controlled, the parents will be notified and the child will have to be removed from the center.

Parents have full access to the building at all times. They must first check in with the After School desk to alert the staff that they are on the premises. This is to protect the other children in the center.

Documenting Incidents, Accidents and Health Issues at Gymnastics Plus

 

 

Dear Parents,

 

Should an accident, incident or health issues occur at Gymnastics Plus you will be notified either by letter or by a phone call.  Any letters given to you by us must be signed and dated so we have documentation that you received a report.

 

Should your child have a fever of 100 or above, diarrhea, vomiting, and lice or any other health issue, you will be notified and asked to come and pick your child up within an hour as to not infect the other children. Failure to do so will result in your child being suspend from our program for a minimum of 2 days. You will still be responsible for your child’s payment during that time.

 

If your child has had the Swine Flu or been exposed to the Swine Flu, please notify us as soon as possible.

When to Notify the Health Department

 

 

 

Gymnastics Plus follows the procedures of notifying the Bay County Health Department when a child has a communicable disease.

 

Communicable disease is something that can be passed from one person to another such as Swine Flu, HIV, TB etc.

 

If a child or employee is suspected of a communicable disease the health department must be contacted immediately. Phone number is 850-872-4455.

Gymnastics Plus Summer Camp 2010

 

Parents,

Please read each statement and initial that you agree to and understand the terms therein.

 

Hours:

Our business hours are Monday – Friday from7:00am-6:00pm. All children must be picked up by 6:00pm to avoid the $1 per minute per child late fee. You must call us if your child is going to be absent.  ____

 

LUNCH/SNACK:

On full days at the Gym your child must either bring their lunch or they can purchase lunch from us. A hotdog meal, Mac & Cheese or Peanut butter and jelly. Cost is $4.00 you must pay the desk person when you bring your child in on a daily basis.  You MUST pay for lunches and snack with CASH. We do not provide lunch. ____

 

We provide morning and afternoon snack and sell snack at the concession stand. ____

 

All snack and lunch money must be paid in cash. ____

 

We are not responsible for any lost or stolen money. Please leave your child’s money at the front desk. ____

 

We will not be able to refrigerate your child’s lunch due to the number of children we will have, it is your responsibility to make sure your child’s lunch is kept cool. ____

 

Dress Code/Storage

Children must be dressed comfortably and appropriately for each day’s activities and weather. Each day your child rotates to different activities including outside. We do not play in the gym everyday. ____

 

Shoes must be worn to our program everyday. ____

 

Children will be provided with a cubby to store their things. Anything left at the end of the day will be moved to the lost and found table outside the lobby door. If your child is missing something please look here. Items will remain in lost and found until the last Thursday of each month, at this time they are donated to the Rescue Mission. ____

 

Cell phones, trading cards, Nintendo DS, PSP’s, Games Boys or other toys are not allowed at the gym.  If you choose to bring them, it is your child’s responsibly.

We have had several items get broken or missing. We cannot be responsible for these items. 

 

Children registered full time will receive a free gymnastics or tumbling class and should wear proper clothing on their gym day. (Nothing with buttons or snaps) ____

 

Sign in/out:

Parents, it is mandatory that you or another adult sign your child in and out everyday. ___

 

You MUST sign your full signature and the time.     ___

Children will only be released to persons included on their pick-up list and who have photo ID. ____

 

Any changes to a child’s pick up list must be made with the desk person. ____

 

Payment:

Snack, lunch and ice cream money must be paid in cash. ____

 

All payments are due on Monday of each week and must be paid by 6:00pm on Wednesday. A $20 late fee is automatically added to all accounts not paid. ____

 

If tuition is not paid by 6:00pm on Friday, we will drop your child from our program. In order for your child to return you must pay all past due fees including late fees, the present weeks fees and a $50 re-instatement fee. ____

 

The registration fee is $50 per child or $35 if they were in our After School Program. ____.

 

Full time fees are $85 per week with a $5 discount for each additional child. The drop in rate is $30 per day per child. ____ or $15 for half day. _____ ½ day hours 8am-12pm or 2-6pm

 

Each child is allowed 1 vacation week during the Summer Camp (June –Aug). You must pay the regular fees for all other weeks to hold your child’s slot.      ____
